1. Registration Form (available online www.sd63.bc.ca/Registration.htm)
2. Proof of child's age (birth certificate or passport)
3. Most recent report card
4. Medical Care Card
5. Proof of address of the student to be registered
6. [if out of catchment, complete an application for Attendance Out of Area form]
